The basis for accepting air medical transport is a carefully completed and submitted electronic transport order (EZT) – https://www.lpr.com.pl/ezt/
After sending the EZT form electronically, it must be printed out, signed, and stamped (as before) in the designated places so that the original, fully completed document can be handed over to the LPR crew who will arrive at your facility. Within a few minutes of sending the EZT, the medical dispatcher at the LPR Operations Center will contact you to perform a medical assessment of the patient for transport. If you do not hear from LPR within 15 minutes, please have the doctor caring for the patient contact the Operations Center at 2222 99999.
The original flight order, together with the order for further ground transport, should be handed over to the LPR team. Each time, the doctor ordering air medical transport is required to consider the risk-benefit ratio for the patient's health and, in case of doubt, contact the medical dispatcher at the LPR Operations Center.
